HMO Plans
An HMO (health maintenance organization) is a benefit-rich product for your employees, while providing you with aggressive long-term cost control. We have HMOs for large and small groups, fully-funded and self-funded.
Your employees are required to see contracted providers for services and elect a primary care provider (PCP). All of our contracted providers are screened and regularly reviewed. This ensures an excellent network of physicians and hospitals for your employees.
With our HMO, no referrals are required to visit a specialist. However, some services require prior authorization.

POS Plans
Our point-of-service (POS) plans are great for large groups wanting more freedom in their health insurance plan.
Like HMO plans, employees should use the Paramount contracted network of providers to receive the best benefits and a primary care provider (PCP) must be selected. However, unlike HMO plans, your employees will have access to providers outside of the Paramount HMO network at a slightly lesser benefit. Employees enjoy the freedom to choose the provider who best meets their needs.
CDHP Plans
We understand that healthcare costs keep rising. That's why we offer our consumer directed health plan (CDHP) to groups of all sizes – including yours.
Our CDHP offers affordable premiums, a large provider network and flexible benefits. Most employers offer a CDHP along with a health reimbursement account (HRA) or health savings account (HSA) to offset some of the cost to employees.
The CDHP network of providers consists of Paramount, HAP Preferred, a consortium of PPOs throughout Michigan and Encore Health Resources. All providers and facilities in this combined network pay at the highest level of benefits. For those employees who want even more providers to choose from, they can elect to go out-of-network at a slightly lesser benefit. They may be subject to potential balance billing.

Alliance Plans
Part of being a local company is collaborating with local organizations. We've partnered with many local associations to bring you Alliance plans.
Our Alliance plans are available to groups with 1 - 50 employees who are fully funded. In addition, membership with one of the participating organizations is required. Your membership gives you access to discounted health insurance rates, as well as many other perks through the organization itself.

HMO
MEWA Plans
Paramount has partnered with Northwest Ohio Business Alliance (NWOBA) to create a self-funded multiple employer welfare arrangement (MEWA) for businesses with 50 or fewer employees, including sole-proprietorships. MEWAs allow small businesses to join together for the purpose of medical benefits as an alternative to Affordable Care Act (ACA) plans.
Paramount offers multiple MEWA plan options with access to ProMedica OnDemand (link) and an integrated care delivery system. Both can reduce medical costs and improve outcomes.

As part of our diverse product choices, our administrative services only (ASO) product option offers self-funded clients a solution to controlling costs, managing risk and responding strategically to critical trends. Our ASO products include a variety of services that are available bundled or separately, based on your client's specific needs.
We provide a multitude of tools to help manage risk while potentially generating tremendous savings. As your partner, we work with you and your clients in every aspect to make sure we're the right fit and the correct risk parameter is selected.
As an ASO participant, your clients will have access to four networks of providers - Paramount, Ohio Health Choice, HAP Preferred and Encore.

Medical Home Program
Our Paramount Medical Home (MH) program is available to large groups only. Based on the medical home concept, the MH program rewards your employees and their providers for working together to achieve better health and wellness.
Employees are incentivized to see their physician and address health conditions that may be present throughout the year. Physicians are incentivized to work closely with their patients during the year to reach their best health.
