Paramount’s Steps2Health Condition Management Programs
As a health care provider, your input and expertise are highly valuable to your patients. One of the best ways to assist your patients is to help them take control of their own health. Paramount’s Steps2Health Condition Management Programs are designed to complement/reinforce the care provided to patients in the office setting. Education, tools, and strategies to manage chronic conditions are offered according to assessed needs.
Each of our programs includes health coaching and case management evaluation and services, pharmacy services and educational mailings.

Over 17 million Americans have asthma and we can help. Paramount’s Asthma Management Program works with you and promotes:
- Use of daily asthma medications to prevent flare-ups
- Sharing personal Asthma Action Plans with family, employers, teachers and coaches

Heart failure affects approximately 5.8 million people in the United States. The most common causes of heart failure are coronary artery disease, high blood pressure, and diabetes.
Paramount's CHF Program promotes:
- Written CHF Action Plans
- Control of blood pressure, cholesterol, diabetes, and weight
- Daily weights
- Formal dietary education, as appropriate
- Visits to a heart specialist, as appropriate
The CHF program includes automatic health coaching or case management services for all members identified for the program. Pharmacy services and educational mailings support are also available.

Almost 20 million people have some form of kidney disease. Millions more are undiagnosed or at risk.
Paramount's CKD Program promotes:
- Formal dietary education
- Blood pressure and cholesterol control
- Control of A1c and blood sugars for members with diabetes
Preserving kidney function and preventing further decline is the main goal of Paramount's CKD program.
When kidney disease is caught early, it's possible to slow, stop, or even reverse it, depending on the cause.
Our program includes automatic health coaching for all Paramount members diagnosed with CKD Stages 3 and available on request to members with CKD Stages 1 or 2. It also includes case management services to all Paramount members diagnosed with CKD Stage 5 or end-stage renal disease. Pharmacy services and educational mailings are also available.

Over 10 million Americans have Chronic Obstructive Pulmonary Disease (COPD). COPD is a serious lung disease that most people get it from smoking. It can't be cured, but it is treatable!
Paramount's COPD Program promotes:
- Pulmonary rehabilitation services
- Smoking cessation
- Written COPD Action Plans
- D Action Plans

Rates of depression continue to increase annually. Over 17 million adults in the United States have previously been or are currently being treated for depression.
High Risk Depression Management Program
Paramount's High Risk Depression program targets members that have experienced an acute exacerbation, have attempted suicide, and/or have a chronic medical condition and depression
This program promotes:
- Visits to behavioral health providers for at risk members
- Adherence to antidepressant medication
- Management of coexisting chronic medical conditions
- Engagement in therapeutic behavioral health services
- Development of coping skills
Our Job as Health Educators is to assess members with mild to moderate depression which would include: situational depression, persistent depressive disorder, and comorbid depression. Development of coping skills and implementation of lifestyle changes are a big part of the depression management program.
Co-morbid Depression Management Program
For people with chronic illnesses, it's often hard to tell the difference between 'understandable unhappiness' and depression. Depression rates and the risk of depression are much higher in individuals with chronic disease. It is one of the most common complications of chronic illness and can aggravate chronic conditions. Often, treatment for depression results in an improvement in medical conditions, better quality of life, and better adherence to treatment plans.
Members in Paramount's Co-morbid Depression program include interventions described in the High Risk Depression Program and additional health coaching calls which include:
- Special periodic assessments to find out how well their depression is being managed
- Encouragement to discuss possible depression plan changes with physicians, as appropriate

There is an epidemic of diabetes in America. About 23.1 million people have diabetes and an estimated 7.2 million people don't even know they have it. That's 9.4% of the US population has Diabetes!! Even worse 84.1 million adults 18 or Older have prediabetes (33.9% of the US population), who potentially could become diabetics in the coming years!
In an effort to combat this growing epidemic Paramount developed a Diabetes Management Program for its members with Diabetes. The purpose of the program is to assist the member in managing their diabetes, through education and recognition that the member is the leader of their own health, at the same time work with members health team (Physicians, Diabetic educators, Dieticians, and therapist) to stay aligned with the American Diabetes Associations Standards of Medical Care In Diabetes. To that end, Paramount's Diabetes Management Program Provides: Health coaching services, case management evaluation and services, pharmacy services and educational mailings.

Paramount's Migraine Program promotes:
- Visits to a headache specialist or neurologist
- Keeping an individual headache diary
- Medications to prevent migraine headaches from occurring

Paramount’s Post Cardiac Program targets members with recent cardiovascular events or procedures, such as:
- Acute myocardial infarction (AMI)
- Coronary artery bypass grafting (CABG)
- Percutaneous transluminal angioplasty (PCTA) with or without stents
- Coronary vessel thrombolysis (treatment to dissolve blood clots in blood vessels)
Paramount's Post Cardiac Program promotes:
- Cardiac rehabilitation services
- Blood pressure and cholesterol control
- Recommended annual screenings
The program is designed to help our members recover and promote heart healthy habits.
The Post Cardiac program includes automatic health coaching or case management services for all members identified for the program along with Pharmacy services and educational mailings.

Ways to Enroll
There are several ways for your patients to be enrolled in one or more Condition Management programs. The primary enrollment source is our disease management algorithm that analyzes medical, pharmacy, and laboratory data monthly to identify patients for our programs. Other identification methods include data collected through case and care management processes, and health management, wellness, or health coaching programs. You may refer your patients to a program and patients may also refer themselves. Members meeting criteria are automatically enrolled but can choose to disenroll at any time.
Condition Management programs can be of great benefit to you and your patients. Not only do they aim to reduce medical costs and acute care utilization, but the main intent of each program is to help members manage their condition, so they can slow or reverse the progression.
