Eligibility
To be eligible for Paramount health coverage through the Health Insurance Marketplace, you:
- Must reside in one of the following northwest Ohio counties: Defiance, Erie, Henry, Huron, Lucas, Ottawa, Sandusky, Seneca, Williams, Wood or Wyandot
- Must be a U.S. citizen or national (or be lawfully present)
- Must not currently be incarcerated
- Must be under age 26 to enroll as an eligible dependent
When to Enroll
Open Enrollment runs Nov. 1 through Jan. 15.
You may also qualify for Special Enrollment Periods outside of Open Enrollment if you experience certain qualifying life events. These include losing your health coverage, moving, getting married, having a baby or adopting a child. Visit Healthcare.gov to see if you qualify for a Special Enrollment Period.
How To Enroll

- Social Security number or document number for legal immigrants.
- Employer and income information, such as pay stubs or W-2 forms.
- If currently covered by health insurance, the policy number.

Individual and Family plans function similarly to employer-sponsored plans in that they are HMO plans, meaning that you will utilize a group of providers for your care who are in our provider network. Additionally, Paramount members can log onto our portal 24/7 to access deductible counters, claims information, your ID card and more.
In an employer-sponsored plan, your employer pays part of your monthly premium. In Individual and Family plans, you will not receive funding from your employer to help pay your premiums.
Monthly payments, co-pays and deductibles are accepted on the member portal.
