We understand you want the best benefits for the least amount of premium possible. That’s why we have a team of dedicated underwriters available to quote new groups and run renewals on existing groups. Their job is to look at the demographics of a group and get you the best rates possible.

Let's get started by first determining how many employees you have. Make a selection below and use the plan checklist as a guide to begin collecting the necessary information for enrollment. Large Group
More than 50 Employees, Fully Funded
To request fully funded rates for a large employer group (51 or more eligible employees), we'll need some information from you first. We've created a checklist (Ohio / Michigan) to make it easier. Information may be sent to your broker/agent or to Paramount directly.
Step One:
Collect the information on this checklist.
- Employer risk assessment form (ERAF) filled out for your group of employees -- usually by a human resources manager
- Census capturing all eligible employees. Please provide as Excel® document. The census should identify:
- Age
- Gender
- Enrollment type (single, couple, etc.)
- Current plan they are enrolled in if there are multiple plans
- Current plan they are enrolled in if there are multiple plans
- Current plan Renewal, current and prior year rates from your current carrier. Please provide these on carrier letterhead. Rate development is required for groups over 100.
- Current and prior year's summary of benefits to help us understand your rate changes from year to year.
- Large claims data with diagnosis for groups over 100 contracts from your current carrier.
- Claims experience by month. Two years is required for groups over 100 contracts.
- Membership by month - for two years for groups over 100 contracts.
- Medical health questionnaires (MHQs) for groups with fewer than 100 enrolled employees. This form must be filled out by each employee. Check with your broker/agent. They may have software available to help.
Step Two:
Submit completed information to your broker/agent or to Paramount directly at paramount.marketing@medmutual.com.
Step Three:
Our team will provide a quote.
Small Group
50 or Fewer Employees (Small Groups)

- Location of the business.

- Smoking status
- Enrollment type (single, couple, etc.)
- Current plan they are enrolled in if there are multiple plans
- Renewal, current and prior year rates from your current carrier. Please provide these on carrier letterhead.
- Current and prior year summary of benefits.
Step Two:
Submit completed information to your broker/agent. He or she will be able to develop the quote for you.
