That's why we have a team of dedicated underwriters available to quote new groups and run renewals on existing groups. Their job is to look at the demographics of a group and get you the best rates possible.

First, we need to know: Is your client a large group (51 or more eligible employees) or a small group (50 or fewer eligible employees)?

Large Group
More than 50 Employees, Fully Funded
To request fully funded rates for a large employer group (51 or more eligible employees), we'll need some information about your client.

Collect the information on this checklist.
- Employer Risk Assessment Form (ERAF)
- Census capturing all eligible employees (Excel® format is preferred). Include age, gender, enrollment type (single, couple, etc.), and their current plan.
- Renewal, current and prior year rates from current carrier. Rate development is also required for groups over 100. Please provide on carrier letterhead.
- Current and prior year's Summary of Benefits
- For groups over 100 contracts, large claims data with diagnosis from current carrier
- For groups over 100 contracts, two years of claims experience by month
- For groups over 100 contracts, two years of membership by month
- For groups with fewer than 100 eligible employees, PDF document (opens in a new tab or window)Medical Health Questionnaires (MHQs). Each employee must fill out this form.
Small Group
50 or Fewer Employees (Small Groups)
To make things easier for you, you can process your small group quotes and renewals through Jet-Insure, our broker portal.
To request rates for a small group, you'll need the following:
- Group information including name, type of business and location of the business.
- Census capturing all eligible employees (Excel® format is preferred) including name, age, gender, smoking status, enrollment type (single, couple, etc.) and current plan.
- Renewal, current and prior year rates from current carrier. Please submit on carrier letterhead.
- Current and prior year Summary of Benefits.
