SilverSneakers®

SilverSneakers® provides seniors with access to thousands of fitness centers across the country, plus in-person and virtual fitness classes. All Paramount Elite plans include this benefit at no additional charge. From weights to machines to group classes led by trained instructors, SilverSneakers has something for any level of ability. Always talk with your doctor before starting an exercise program.

CareSignal®

We are excited to offer select Paramount Elite Medicare members a free program called CareSignal®. This program sends you simple text messages or automated phone calls to check in on your heart health or breathing. If you’re selected to be in the program, you’ll receive a call from CareSignal.

Condition Management

Paramount’s Steps2Health Condition Management programs help you take control of your health by offering you and your healthcare provider support and resources. 

Available Condition Management Programs 

Paramount currently offers programs to help manage the following conditions:

  • Asthma
  • Congestive heart failure
  • Chronic kidney disease
  • Chronic obstructive pulmonary disease
  • Co-morbid and high-risk depression
  • Diabetes
  • Migraine
  • Osteoporosis
  • Post-cardiac event

These programs are designed to help you slow or reverse the progression of your health condition, as well as support your overall health.

Each of our programs includes:

  • Health coaching and case management evaluation and services
  • Pharmacy services
  • Educational mailings
  • Web-based interactive tools
  • Online support

Enrolling in a Condition Management Program

If you are interested in enrolling in one of our condition management programs, please contact Member Services toll free at 1-833-554-2335. If you use TTY equipment, please call 711. From April 1 through September 30, we are available Monday – Friday, 8 a.m. – 8 p.m. From October 1 through March 31, we are available seven days a week, 8 a.m. – 8 p.m.
