Preventive Health with Perks

Your primary care provider (PCP) is your partner for good health. Following your PCP’s recommendations for preventive health services helps identify health problems earlier, when they are often easier to treat.

To get rewarded for taking care of your health, meet the requirements below by Dec. 31.

All Paramount Elite members who complete either an annual wellness visit with their PCP or an in-home health assessment with our vendor Porter, as well as two activities from the list below, will earn $50.

• Basic metabolic panel
• Bone mineral density testing
• Breast cancer screening (mammogram)
• Colorectal cancer screening
• Flu shot
• Pneumonia shot

Paramount Elite members with a medical diagnosis of diabetes who complete three activities from the list below will earn an additional $30.

• Diabetic eye exam
• Diabetes self-management training
• HbA1c testing
• Kidney health evaluation

How do I get started?

Members are automatically enrolled in our wellness and diabetes rewards program(s). You’ll automatically earn credit for the eligible activities through your claims, so make sure to use your Paramount Elite insurance when completing an eligible activity.

How do I get my reward?

Your incentive will be added to your NationsBenefits^® Flex Card, the same card you use for your over-the-counter benefit. Please keep this refillable card as long as you remain a member in your plan, because your wellness reward will be added to it each year. Please be aware that you won’t receive your reward immediately after completing your eligible activities; it can take several weeks.   

Will my reward expire?

It will expire 12 months after being added to your card.
