Annual Wellness Visits vs. In-home Health Assessments

Annual Wellness Visit with Your Primary Care Provider (PCP)

Your PCP is your trusted partner on your health journey, so it’s great to check in with them for an annual wellness visit.

During this visit your PCP will: 

  • Take routine measurements like height, weight and blood pressure
  • Review your and your family’s medical history
  • Review your current medications
  • Build a preventive care plan for the year

You can schedule this visit anytime during the calendar year, but the earlier you complete your visit, the sooner you can plan for any other preventive care you may need.

In-home Health Assessment with Porter

During this visit, a licensed healthcare provider from our trusted vendor Porter will visit your home and:

  • Take routine measurements like height, weight and blood pressure
  • Discuss your medical history and current conditions
  • Complete important tests such as bloodwork, diabetic eye exams, bone density tests and more (if needed)

Consider scheduling an in-home assessment if you’d benefit from completing this visit in the comfort of your home. It provides one-on-one time to review your health and is designed to complement, not replace, the care you receive from your healthcare team. At the end of the visit, you’ll receive a summary of your results to discuss with your PCP. 

Virtual visits are also available. If you complete a virtual visit with Porter, they will send your results in the mail.
