Chronic Kidney Disease (CKD) Management Program

Preserving kidney function and preventing further decline is the main goal of Paramount's CKD program. When kidney disease is caught early, it's possible to slow, stop, or even reverse it, depending on the cause.
Paramount's CKD Program promotes:
- Visits to a nephrologist (kidney specialist)
- Formal dietary education
- Blood pressure and cholesterol control
- Control of A1c and blood sugars for members with diabetes
Our program includes automatic health coaching for all Paramount members diagnosed with CKD Stages 3 or 4 and available on request to members with CKD Stages 1 or 2. It also includes case management services to all Paramount members diagnosed with CKD Stage 5 or end-stage renal disease. Pharmacy services, educational mailings, web-based interactive tools and on-line support are also available.
