Case Management

Case Management is a program where nurse case managers work with members, doctors and medical providers to coordinate care. Case managers can teach you ways to play a bigger role in your health care. Examples of conditions that may qualify for case management include:

  • Asthma
  • High-risk pregnancy
  • Teen pregnancy
  • Newborn babies with serious health complications
  • Uncontrolled diabetes
  • Cancer
  • HIV
  • Severe mental health or substance abuse disorders
  • Organ transplant
  • Spinal cord injuries
  • Severe trauma

A request for care management services can come from the member, a family member, a medical provider, or from claims information.
