We are committed to keeping you as healthy as possible and improving your quality of life. One way we’re able to do that is through our Steps2Health Condition Management programs.

We currently have nine programs to help you achieve your best health. Each of our programs includes health coaching services, case management evaluation and services, pharmacy services, educational mailings, web-based interactive tools, and online support. Also, every program has been designed to support the care you receive in your doctor’s office, so we’re always working in conjunction with what your doctor has prescribed as your best course of care.

There are several ways to enroll in a Condition Management program. You may be referred to a program by your physician, or yourself. We will also monitor claims, pharmacy, laboratory, utilization and electronic health record data for potential enrollment in the program. You may also be enrolled through Case or Care Management  programs, as well as other Paramount services such as wellness and health coaching programs. If you meet the criteria, you will be automatically enrolled in the appropriate program, but you may choose to disenroll at any time.
