Paramount’s Steps2Health Condition Management programs are designed to compliment/reinforce the care provided to patients in the office setting. Education, tools and strategies to manage chronic conditions are offered according to assessed needs.
Each of our programs includes health coaching and case management evaluation and services, pharmacy services, educational mailings, web-based interactive tools, and online support. Our programs provide support and resources for patients with:
- Chronic Heart Failure (CHF)
- Chronic Kidney Disease (CKD)
- Chronic Obstructive Pulmonary Disease (COPD)
- Co-morbid and High Risk Depression
- Post Cardiac Event
There are several ways for your patients to be enrolled in one or more Condition Management program. The primary enrollment source is our disease management software application, MDS, which analyzes medical, pharmacy, and laboratory data monthly to identify patients for our programs. Other identification methods include data collected through case and care management processes, and health management, wellness, or health coaching programs. You may refer your patients to a program and patients may also refer themselves. Members meeting criteria are automatically enrolled, but can choose to disenroll at any time.
Condition Management programs can be of great benefit to you and your patients. Not only do they aim to reduce medical costs and acute care utilization, but the main intent of each program is to help members manage their condition, so they can slow or reverse the progression.
