Welcome to Paramount's Aligned Network Instruction Page

Paramount has created a business relationship with Private Healthcare Systems (PHCS) to provide our Paramount Employer Select, HMO Plan, and Paramount Maximum Choice (PPO) members with a more expansive choice of health care providers outside the immediate Northwest Ohio area.
When searching the on-line directories for a provider, please be sure to choose the Aligned Network that is associated with your product offering (see box below). When viewing the PHCS On-line directory be sure to choose the Healthy Directions Network in the drop down “Choose a Network” Screen.
In order to maximize your level of benefits, utilize participating providers in either the primary or Aligned Network.

As noted above, the Aligned Network is only available if the provider is located outside the Northwest Ohio area. Please note that the Aligned Network directory will contain some providers in the Northwest Ohio area, who will be considered Non-Network, based on the product you are enrolled in. Use of these providers will result in your claims being paid at the lowest benefit level, or not covered at all. These providers are located in Ashland, Crawford, Defiance, Erie, Fulton, Hancock, Henry, Huron, Lenawee, Lucas, Monroe, Ottawa, Paulding, Putnam, Richland, Sandusky, Seneca, Williams, and Wood Counties. A sample of these hospital facilities are defined as follows:

Mercy Hospital of Defiance
Mercy Hospital of Tiffin
Mercy Hospital of Willard
St. Anne Mercy Hospital
St. Charles Mercy Hospital
St. Luke's Hospital

I confirm that I understand and agree that the providers who are located in Ashland, Crawford, Defiance, Erie, Fulton, Hancock, Henry, Huron, Lenawee, Lucas, Monroe, Ottawa, Paulding, Putnam, Richland, Sandusky, Seneca, Williams, and Wood Counties, and are also listed within the PHCS Healthy Directions online diretory, are considered out-of-network providers within the Paramount Premier Plan Product offering(s). Furthermore, the use of the PHCS providers in the 19 counties listed above will result in my claims being paid at the non-network level of benefits.
